New York COVID-19 Vaccination Plan
Dear Urgent Care Center Administrators:
The New York State Department of Health (NYSDOH) is now enrolling Urgent Care Centers (with sites located outside the five boroughs of NYC) in the New York State COVID-19 Vaccination Program. Urgent Centers (outside NYC) interested in administering COVID-19 vaccine must enroll in the NYSDOH program to be ready to order and receive publicly supplied COVID-19 vaccine and ancillary supplies when the vaccine becomes available.
CLARIFICATION: One of these documents below incorrectly advised organizations to send these documents to an email address designated for questions. To enroll in the COVID-19 Vaccination Program providers must complete the application in the Health Commerce System online tool. Please enter the Provider Agreement and Profile information in the online Health Commerce System application tool. The deadline for enrollment has been extended to close of business on Monday, November 30, 2020. Any questions about the enrollment process should be sent to COVID19Vaccine@health.ny.gov

Note: The New York City Department of Health and Mental Hygiene (NYCDOHMH) has a separate process for enrolling providers within NYC. Providers with locations both within and outside NYC will need to enroll in both the NYC program and the NYSDOH program.
